Health Insurance Claim Process in India: Step-by-Step Guide
Complete guide to filing health insurance claims in India — cashless and reimbursement process, documents, and tips to avoid rejection.
Understanding Health Insurance Claims
Filing a health insurance claim correctly can mean the difference between full reimbursement and significant out-of-pocket expense. Understanding the process before you need it — ideally before hospitalization — is essential. India has two types of claims: cashless and reimbursement.
Cashless Claims (Network Hospitals)
Cashless claims are the preferred option as you don't need to pay upfront (except non-covered items):
- Verify network hospital: Check your insurer's website or app for empaneled hospitals
- Planned hospitalization (72 hours before): Contact insurer's TPA desk or call the toll-free number to get pre-authorization
- Emergency hospitalization (within 24 hours of admission): Notify insurer immediately
- At the hospital: Show health card/policy document + photo ID at the insurance desk
- Hospital submits cashless request to TPA/insurer
- Approval received (usually within 4-6 hours for planned; 1-2 hours for emergency)
- At discharge: sign discharge papers; insurer pays hospital directly
Reimbursement Claims (Any Hospital)
- Get treatment at any hospital
- Collect all original bills, receipts, prescriptions, discharge summary
- Submit claim form + documents within the stipulated time (usually 30 days of discharge)
- Insurer/TPA reviews and verifies
- Amount credited to bank account within 15-30 working days
Essential Documents for Claims
- Duly filled claim form (from insurer's website)
- Original hospital bills and payment receipts
- Discharge summary with diagnosis and treatment details
- Doctor's prescription and investigation reports
- All pharmacy bills
- For accident: FIR if applicable
- Bank account details for reimbursement
Why Claims Get Rejected (and How to Avoid)
- Pre-existing condition: Waiting period not completed — read policy carefully before buying
- Excluded procedure: Cosmetic surgery, fertility treatment, etc.
- Late intimation: Always notify insurer within required timeframe
- Incomplete documents: Keep all originals; pharmacies can provide reprints
- Policy lapse: Pay premiums on time; set auto-debit
Escalating Rejected Claims
First: raise grievance with insurer's grievance cell. If unsatisfied, approach IRDAI (insurance regulator) at igms.irda.gov.in. Further escalation: Insurance Ombudsman in your jurisdiction (free, fast, binding decisions up to ₹30 lakh).
