Menstrual Health: An Overview

Menstrual health is a core part of reproductive and overall health. Despite being experienced by half the population, it remains a taboo subject in India. This guide provides clear, accurate information.

Normal Menstrual Cycle

  • Cycle length: 21–35 days (average 28 days). Count from first day of one period to first day of next.
  • Period duration: 3–7 days
  • Flow amount: 30–80 ml total per period (3–6 soaked pads/day is normal)
  • Colour: Bright red to dark red/brown. Dark blood at start/end is normal (older blood)

Common Period Problems

Dysmenorrhea (Period Pain / Cramps)

  • Primary: Normal prostaglandin-induced cramps. Common in teens and young women.
  • Secondary: Due to underlying conditions (endometriosis, fibroids). Gets worse with age, not better.
  • Relief: Ibuprofen (if no contraindication) before pain starts; heat pad on lower abdomen; gentle yoga (child's pose, cat-cow); avoiding caffeine during period

Irregular Periods

Cycles that vary by more than 7–9 days, very long cycles (>35 days), or very short (<21 days) should be evaluated by a doctor. Common causes: PCOS, thyroid issues, stress, extreme weight loss/gain, excessive exercise.

Heavy Periods (Menorrhagia)

  • Soaking a pad/tampon every 1–2 hours, passing large clots
  • Common causes: Fibroids, adenomyosis, hormonal imbalance, copper IUD
  • Leads to iron deficiency anaemia — common in India
  • Consult a gynaecologist if experiencing heavy flow

Missed Periods

Other than pregnancy, common causes: stress, PCOS, thyroid, sudden weight loss, over-exercising, premature menopause. A missed period deserves investigation if not pregnant.

PCOS: The Most Common Hormonal Disorder

  • Affects 1 in 5 Indian women
  • Symptoms: Irregular or missed periods, excess facial/body hair, acne, weight gain around waist, hair thinning on scalp
  • Diagnosis: Blood tests (LH:FSH ratio, testosterone, insulin) + ultrasound (ovarian cysts)
  • Management: Low-GI diet, exercise, weight loss (even 5–10% helps), medication (metformin, OCP) as prescribed
